The album was recorded at Miraval Studios, Brignoles, in early 1990 and mixed at Wisseloord Studios, Hilversum, the Netherlands later that year. It was produced by the band and Tom Allom and the first time since 1976's Sad Wings of Destiny that Judas Priest and Tsangarides worked together.

Despite the album being finished in March 1990, the album's release was delayed due to the pending, much-publicized subliminal message trial that began on 16 July 1990. The band was the subject of a civil lawsuit alleging their recording was responsible for the suicide attempts of two young men in Reno, Nevada on 23 December 1985. The case was eventually dismissed on 24 August 1990. With the trial behind them, the band finally released the album on 3 September 1990.

The album was certified Gold by RIAA in January 1991, also received a Grammy nomination for Best Metal Performance at the 33rd Annual Grammy Awards, losing to Metallica's cover of the Queen song "Stone Cold Crazy".

It is the first Judas Priest album to feature drummer Racer X and with his heavy use of double kicks it gave Judas Priest a new sound and heavier edge.

A remastered CD was released in May 2001, including a live recording of "Leather Rebel" and a previously unreleased song, "Living Bad Dreams".

Following the tour for this album, singer Rob Halford left the band in May 1992 and maintained little with his former bandmates throughout the 1990s.

      Unleashed upon Earth in September 1990, and still All Guns Blazing with "lightning in the dark" an incredible 35 years later, Judas Priest's "Painkiller" is absolutely molten metal which threw down a hell of a gauntlet for a new decade. Right from THAT ferocious drum intro (take it away new boy Scott Travis) through to the fittingly epic finale "One Shot at Glory", it just grabs you by the throat and pummels you nonstop for 46 minutes.

      This original Australian pressing is one of my most prized bits of wax - it's ball-tearingly, face-punchingly heavy, cut loud but not hot, and clear as crystal. I also have the red/black marble Aussie pressing from 2020, by comparison that honestly sounds like pretty weak tea to my ears. This one is my go-to.

      All together now: "PAIN! PAIN! KILLER! KILLERRRRRRRR!!!"

        Just picked this up and gave it a cleaning and listen. Holy crap this sounds good on my system! Loud as Hell, quiet and flat vinyl. What more can you ask for. I saw this concert many moons ago. Ah, the memories

          Sounds great to me! Cranked the hell out of this through my Forte IV’s and it has great sound, quiet between songs, no complaints on the dynamics or overall sound. I’d imagine an elliptical cart would have a hard time with IGD as this is cut pretty close to the center, my 740ML tracked it well however. No complaints for the price!

                  What a sloppy bad cover design. Same picture on all sides. There is no group photo in the original. It is not clear which is the an and b side of the record. Because of such bad press, I'm losing interest in collecting records.

                  The saw pattern in the middle of the CD could have been applied to the paper in the middle of the record as well. But where is this creativity and vision? There is a logic of releasing it to the market as soon as possible and making money.
